I’m writing a book and I want your input. I’m asking you to take some time to fill out an anonymous survey. The information you provide will be extremely helpful.
By answering survey questions and submitting your responses, you agree to the following:
- You are filling out this survey voluntarily.
- You may choose not to answer any questions that you do not want to answer. You may also choose to provide as little or as much information as you want.
- Your answers are anonymous. You will be asked to provide basic demographic data such as your age and gender, which you may refuse to answer. You will not be asked for personally identifying information. Your IP address will not be logged.
- You understand that after you submit your survey responses, there will be no way to remove or delete your responses.
- You understand that the only people who will see the full survey results will be me and my co-author.
- You agree to allow me to use your answers in aggregate form. This means basic data such as age or gender may be combined with the results from many surveys.
- You agree to allow me to quote any of your responses, in whole or in part. You understand that, because this is an anonymous survey, you will not be identified in any way and will not receive credit for your quotes.
- You agree that if I quote your survey responses, any names of people or places that you provide will be changed to further protect anonymity.
